Filling out this form allows us to verify your SilverSneakers eligibility, link your 16-digit SilverSneakers ID to your account, and ensure you are registered correctly for our senior activities.

Once the form is submitted, please bring your SilverSneakers ID with you on your first visit so it can be scanned at the front desk.
